Annual Preventive Wellness Exams
The Wellness Exam includes an annual wellness screening, labs, vaccines & provider consultation. It may qualify as an annual wellness visit for your insurance carrier’s incentive plan. Community Care members are now accepted.
The results from your personal health screening are made available a few days following your exam following their review by a physician. You will receive a personalized email containing a link to the Total Wellness patient portal where you will be prompted to log in using your personal identification code.
Once logged in, the portal will display an intuitive look at your personal health status, complete with a color-coded risk analysis review and links to receive more information on what the results mean for your health.
If you qualify, your insurance plan will cover 100% of the wellness screening. This means you pay nothing (no deductible or coinsurance). Without insurance, it will be $99 (cash only).
YMCA members who complete a wellness screening will receive a $25 VISA gift card.
